*Please note there are no ‘roll-overs’ – you must take advantage of the opportunity for a service within a particular calendar month, and if you do not, you forego that opportunity for that service in the month)

For our founding members:

A. 20 units of Botox^® twice per year (minimum 4 months between treatments – limited to first 100 people in the first year only)

B. Welcome Bag (full-size products – limited to first 100 people in the first year only)

Subscription Fees:                      

1. The annual Premier Membership Fee is $4,200.
2. Subscribers paying the full Subscription Fee in one lump sum receive a $300 discount and pay only $3,900.
3. Subscriber’s electing a monthly payment plan hereunder will pay $350 per month.
4. All Subscription Fees are non-refundable and non-transferable.
